Webdesign and logo for webpage providing health services

We need to create a logo and a graphic design for a webpage focused on health services. We have already a vision for the look of our logo and wireframe of the page (see attached files). We need only graphic design in PSD format and the logo like a vector graphics, we don't need any HTML/CSS implementation or any programmer services.

About logo - The basic idea is on the picture. We want to have our logo in 3D - Four squares should be in fact cubes, but these cubes has to be placed so that the cross symbol between them has to be clearly visible and recognizable.
Title "Lékař Plus" can be 3D or 2D with some shadow or mirror efect and with the nice, (maybe abstract), but decent background. This is my first conception and You can change it, if you show me better one. The logo has to be original and looking easy but professional.

The webpage is primary in the czech language, but it will be translated to other languages very soon. The language menu is on the wireframe on the top right corner. The language menu should be made as a list of small flags or something like that, but my mind is open to Your creative ideas.

Top menu is like a classic menu in windows programs, first layer is displayed "inline" and "subitems" are displayed after focus on the item in the root of the menu.
Searching bar contains text field for text and the button.
Video/text box on the left side can contains text or video or both - the width and the height are not fixed yet, you can fit it if you need.
Content is content :) Images, text, links,...
Tags - here the searched parameters from search bar will be displayed in the list with possibility to remove them from search conditions (you can use the symbol of the cross or any other symbol with similar sense).
2 connected icons are 2 buttons with the special meaning. One of the button is link to the map and the other button is not specified now.
The form on the bottom right corner is an general form, where we need to design form inputs (radio buttons and checkboxes can be with the default system look) and buttons and the whole form. This elements will be used in all of the forms on the website. :)
Other blocks don't need comment, I think, but You can ask me for anything, of course.

The basic colors are on the picture logo.jpg, but these colors are not fully binding and you can change it a little if you need.

The design should look easy, modern and professional.
