Video recorder added to form

Hi Alex

My test site is http://www.taylor-strauss.com/Test/. Under candidate you have the option to submit resume. I have added some shortcode for a video recorder plugin to the page in "edit page" in wordpress dashboard ([vidrack align="center"][submit_resume_form]) but the problem is the video recorder option extends to the next preview page and success page as well. I would like to add the (record video button) to the form under the current (photo - choose file) button (image example attached). Their website explains this for adding a field to the form. but I can't get this to function. I know it's a small tweak I cant figure out. I would be happy to pay for full hour if its a small quick fix.
