User experience designer needed to help redo some job posting forms

I want to redo some of the forms on my job advertising website to make them more usable and also encourage conversions.

I would like to work with someone who is experienced in web usability and is also highly commercial. To start I want to redo a set of forms which is currently split into 5 simple pages.

What I want is wireframes for these 5 pages (maybe even cutting them down into less pages) which make the process easier and also help increase conversions, I have a web team who can then implement so all I need is the wireframes/design.

If I can find the right person I want to review the usability and user experience journeys of our entire website.

I'd like to see examples of UX work completed for commercial B2B websites. Any examples of work with forms which are also part of a buying process will be great to see.
