Upgrade Newsletter Form On Website

I have a newsletter form on my website which I want to improve.

I want to add green ticks and text down the side of all pages and posts (apart from homepage), which will give the benefits to signing up to newsletter.

I want the green ticks to be within a box which also contains the newsletter.

The Newsetter already links into my aweber campaign, so I just need the green ticks to be added.

Here is an example of what I mean

Look down the right hand side and you will see the newsletter sign up and green ticks.

As you can see there is not much inducement for them to sign up to the newsletter.

Please provide an estimate of how long it will take, and the price.
