True - False Quiz for a website

Hi,

we are looking for a developer/designer who can create a interactive quiz for us which then should go up on a website.
The layout should be similar to this one: http://theoatmeal.com/quiz/sriracha_addict

The quiz is about real and fake herbs (we have the list ready). So it would be great if we can have a small herb illustration as a header image as well.
