Worldpay expert needed - Connect donation form to a Worldpay Payment Page

I require a form (that has already been built in HTML and CSS) to link with a Worldpay account 'Hosted Payment Page'.

The form gives the user the option to state an amount of money they would like to donate. Depending on whether this is single payment or direct debit the form to different setups of the 'Hosted payment page' to process these different options

The following fields are on the form:

• Single payment or direct debit option
• Amount payment boxes (£10, £20, £50 and £100)
• Contact details (email, address)
• Privacy checkbox
• Gift aid approval checkbox
