Wordpress form integration , data saving & extraction in SQL

Programmers good at PHP| CSS|SQL required for the project of website partial development in Wordpress, the scope of the work is 5 to 6 form predesigned layout to be done, all 5 forms almost similar content to be integrated in Wordpress
1. with the the form data needs to be stored and extracted as required
2. The form should have auto numbering feature for the sl . no.
3. The retrieved stored data to be displayed as lists format also individual data format that needs to be e-mailed or printed as required.
4. The retrieval can be through search of sl. no. or through the course application etc.
