Wix: display database collection

We are building a website on Wix. We have created a Wix database (collection) with three fields: Name, photo, story. Now we want to make Query to search and collect data from that database.

1. Create field where the user inputs a "Name" (for example "Kate").
2. Create a search field, where the user can type in any name. Then click on a button "Check".
3. If the name matches with a name on our database (for example if "Kate" exists and is an exact match)
=> then show the following message: "< then show: "< is NOT blacklisted"
4. If the name matches with a name in our database collect and show all the data for that record (show all three fields "name", "photo", "story" for that Kate)
