Website Maintenance (Graphic Design and PHP Form)

We would like a few additions to our site, as follows:

1. Add two phone numbers to our Dreamweaver site template.

2. Add 4 graphic buttons to the site template. The buttons will need designing and will need to link to various pages within the site.

3. Add various logos for payment methods and banks to the site template. Also add the SRA logo (can be supplied).

4. Create a small php form on a template page for a call back feature. Form to include Name, Phone Number, drop down for best day to call and drop down for best time to call. On submission the form should be emailed to our office. Further details and examples can be supplied if required.

5. Add an About Us tab to the template. This page will need a template of it's own so that we can add some info about the company and also some staff profiles.

We have a rough mock up of where everything should go on the page, and can supply this on request.
