Webpage editing

I have a webpage where the following needs to be looked at:

a) Page 1 - change question radio buttons to drop down list (include a null option called 'Select...')
b) Page 1 - change phase radio buttons to drop down list (include a null option called 'Select...')
c) Page 2 - option to add in second parent details and include all fields (apart from address fields)
d) Page 3 - change radio button to drop down list (include a null option called 'Select...')
e) Page 3 - validation for new school date can only be greater than or equal to today's date
f) Page 3 - use this date picker for all date fields (http://t1m0n.name/air-datepicker/docs/)
g) Page 4 - if the sibling's option is ticked, then to show a box with the following fields
Sibling's first name, Siblings surname, Sibling dob (ideally for it not to mess up the current layout, so placed as best as you can see)
h) Page 4 - Option to show / hide information for preference 2 & 3 (just so it looks a bit cleaner)
i) Page 4 - For second & third school preference change to list like school preference 1
g) Page 5 - on the review page if any of the tick boxes are checked = Yes, else = No
h) Page 5 - all of the details which are entered on the pages prior, to be shown on the review page
i) Page 6 - once the Submit button is clicked, for some type of spinner to appear (before the response comes back from the RESTApi)

One other thing, in Chrome & Safari the date is when entered into the date of birth field looks a bit strange... Chrome = dd/mm/yyyy, however in Safari the date is mm/dd/yyyy. There is no issue with the dates appearing on the summary page as I've checked this for both browsers.
