I need an Upload form to allow 2 photographs to be upload on a Ratings form.
The form is a Loo Ratings Form for visitors to review the disabled toilets they have visited.
The form consists of Name, email address and Loo Location, these are 1 line text areas.
A ratings box, this offers in a drop down box 5 choice of which only 1 can be chosen.
A Facilities selection, this consists of 6 check boxes, any or all may be selected.
A Cleanliness box, this consists of 4 choices of which only 1 can be chosen.
A Photo Uploaded box, which is either Yes or No is chosen.
A Brief Description box, for a description of the loo is written.
A General Comments Box, for general comments.
At present I use 2 separate forms and would prefer it to be one form.
