Transport Register System web-based or access database

Attached is JUST the series of form and how each form should work along in excel, select the developer tab from the menu bar and you will see all the forms. BUT i do not want to do this in excel as it requires multiple users to access the system at the same time.

Template for word documents will be provided if you are successful.

I need to translate this into a working solution, the business I work for does not have MS access database but I believe there is a free solution access enterprise or something like that.

User rights need to be set

A quick turnaround within the week is required without a significant amount of strain on my time.

Please provide examples of work you have done before.
