Simple secure web form and writing to database

Hi Guys,

Here we are please can I have cost (and timescale) of turning attached into a hosted web form.

Requirements are as follows

• It needs to be hosted on a secure https server (we will buy a url for this)
• The access to the form must be username and password protected
• The form data must write to a database
• The data must be accessible (downloadable via Excel/CSV/Access)

Nice to have
• Up scalable option to add more forms
• Dashboard for data collected

Any suggestions welcome.
