SharePoint Design forms - keep it simple

I need someone to design some nice looking sharepoint forms and pages

What’s important to note is whilst I am admin of the ‘site’ I don’t have access to the server farm, so you will need to advise me if some of this is not possible before you submit your proposal.

Just looking to keep very simple

Sharepoint is locally hosted at my workplace.
