Set up Heart Internet Reseller area on my domain

I wish to have a reseller area set up on my own domain name.

I am using Heart Internet and the info required for the job is here:

The domain name is purchased and hosting set up already, I will provide the package information, but I\'d like a page set up in this style, using my logo (will be supplied) and my packages:
