We need to program the attached form in php for inclusion into a WordPress site for a travel agents that we are currently developing. On submit the form will send a simple text email through to the client with all the captured date included.

There are a number of calculation sliders included which will allow the user to specify various requirements.

We can give you access to a development WordPress install so that all code, templates etc. can be modified and created in a real environment. We can style the resulting page, so no need to worry about design. Just the layout and functionality.
