React app to showcase NPM file uploader

I need a small React application built to showcase this package: https://www.npmjs.com/package/react-simple-file-upload.
I will send a codepen with an API key if we decided to work together.
Requirements:
- A React application using create-react-app that contains a form. The form elements should be Name, Password, Password Confirmation, and Avatar.
- Avatar is added by using the Simple File Upload component.
- The form should be visually pleasing. I don't care exactly what it looks like, but it needs to look professional.
- The form should accept inputs and submit those inputs to a backend API such as Firebase.
- The backend doesn't have to be functional, the purpose of this app is to show the user how to get the data and submit it somewhere.
