PHP reporting to screen, print and PDF/CSV export

I need a reporting framework which will allow me to take data from an HTML form and display the results in table form OR chart form depending on the report I have built, print the results in a printer friendly manner and export to PDF and CSV.

The script needs to be flexible to allow me to have any number and type of fields on the input form and to allow for users clicking all, none or some of the options - e.g. user select nothing on form so the query is "select * from table" or user selects 1 field on form so query is "select * from table where name='Bob'" and so on to the number of options on each form.

When the results are displayed I then would like the user to be able to print the results with formatting so it looks like similar to how it does on the screen and I must be able to add logo, headers and footers to each page.

Also user should be able to choose to export to CSV and PDF - for PDF the PDF file should also look similar to the screen version (i.e. not just a plain table dump) and the PDF should have logo, headers and footers on each page.

I have got very close to getting this to work myself using PHPExcel, TCPDF, Flot Charts and Datatables but I cannot quite get there and it is taking much too much time to try and resolve so I want someone else to do it for me. I do not mind what libraries you use as long as the functionality works though I would like to keep using Flot if possible.
