PHP form to collect data, take photo and print (Android iOS)

Im looking for a php based form which can collect key information from a user, take a photograph using the built in camera, save the record to a MySQL database and print to a network printer.

The output is a pdf style visitor badge which is dynamic and displays certain information from session state entries (I can provide this, session state information etc).

The form is to be supported on Android and iOS tablets.

Im looking for assistance in creating a user form which will collect a number of data fields and then store these into the database, the framework / infrastructure is already in place.

The form would ideally also work in an offline situation where if there was no internet it could capture the information (excluding photo) and then upload to a temporary table on re-connection.

The "module or form" is stored in a separate subfolder on the web infrastructure, on successful passing the URL parameter it logs the user in and automatically send the device to the subfolder, this login ensures that the device pulls all the relevant connection strings, session states, parameters etc
