Php access to timed subscriptions

I need reusable PHP code that will enable me to:
1. Assign usernames and passwords to subscribers
2. Must cater for multiple subscriber types or categories
3. Limit access to data in a database (MySQL) based on subscription type or category.
4. Login’s must timeout (15 minutes inactivity - nice to have) and must be valid for the duration of the session (logout if browser is closed - required)
5. Subscriptions (and associated logins) must only be valid for a pre-defined subscription period.
6. Must be standalone and NOT rely on any CMS.
7. Fixed price quote please not a time and material quote.
8. Must be accompanied with detailed instructions on how to use in user created HTML and now to setup the database to work with the code.
