Pdf Form Design (Dynamic)

I am looking forward to get a dynamic pdf form designed. It should comprise 3 different sections, the user will be jumped to the right section as he selects the option on the first page.

No calculations are involved, some display, adding or hiding field may be required.

Finally, when the form is finished, the user should press the submit button.

It should be emailed to our desiginated email address. Now the main part is - it should not open the user\'s outlook or ask him to attach to a web based email application after saving the form. It should automatically process the data and email to us. If it needs any database on our hosting, we can do so.

We need to have that form delivered to us in 2 days time.
