We are looking for a developer to enhance our current Order Form we have created. It was produced using Jot Form and whilst it contains the features we require, we have realised it will not allow us to produce all the functions and capabilities we hoped for. We have got it to a stage that is allowing us to use the form, however we now require a developer to take it to the next level.
Our current form only contains one of our Sports (Rugby), we do many other sports & a vast range of leisure items, however we have realised this platform (jotform) will not be able to cope with our full product range and various constraints each one will contain.
We will provide you with our current form so you can see the functionality.
You will be provided with full details of each product and the constraints they contain, to enable you to produce the form.

Description of requirements/features: Overview of the form requirements:
• The form will contain every Sport (6 sports), Leisure items (30 + garments) & Stock items we offer (30+ products)
• The user will be able to select their product within their first section (eg. Rugby) and then add their products from another section (eg. Add leisure items), the form will save the details submitted and combine together as 1 submission once complete. This then needs to be automatically sent to an (multiple) email account once the user clicks the ‘submit button’. (We have managed to get our current form to partly do this function, but it is not as smooth as we would like & obviously this is only for 1 sport - Rugby).
• The user should have the functionality to go back a step if they make a mistake and edit their choice accordingly (our current form has bugs when doing this, it forgets the constraints set up & effectively allows the user to now choose from options that are not available).
• Must contain multiple options with conditional logic (the question answered will determine the next set of questions that appear for the user to select from).
• Mixture of Radio Buttons/Drop down options/free text fields/calculator widgets/upload multiple files.
• We would like an image preview of the product to appear once the user selects their design from the drop down field.

Maintaining the Form:
We would like the flexibility to be able to edit the form once it has been built. We are a bespoke manufacturer and products constantly change features (eg. Fabric, colour, design changes). It is vital that we are able to do these small edits ourselves without having to wait for a developer to edit on our behalf.

• Sales Representative name
• Product (introduce a new product which then allows us to choose which fields we need present, eg. Design/Fabric/Fit/Garment Colour).

• Sock Type & Options
• Sizing Grids
• Decoration Options
• Name & Number Options
Most of the above fields are drop down fields, our current form is very easy for us to amend if we need to change any of these features. These types of changes can happen on a daily basis, so it is definitely something we need to be able to do ourselves.

Future enhancements to the Form:
We have a vision of where we hope to take the form too, therefore we would like you to build this with the following features in mind:
• Link into Sage (the form will need to email directly to our sales person upon submission and feed straight into our management system as a new order with the next corresponding order number).
• Contain pricing for each product, with the functionality to apply individual customer discounts (linking into our Sage system).
CMS and Admin requirements: The ability to add new products behind the scenes.
Products to be stored in a SQL database
Ability to batch upload products from a CSV and download via CSV

Extra notes: We're after a developer who can create an online order form, with an administration form (CMS) for a team to manage and modify the look and feel of the form, along with the ability to apply conditional logic to enable set fields to appear or display lists according to previous selections.

In addition to this the output of the form must save the data in a PDF format, along with being stored centrally within the database and feed through the information into another system via API (Trello or Sage).
