Need to create a quick HTML5 Form using Telerik Controls from existing PDF/ExcelForm.

Description of every page/module: Here's what I need to be done:

Step1: Need to create a HTML5 Medical Chart form from using Telerik's KendoUI Controls using an existing PDF or Excel form which would be given to you.

Step2: Need to highlight the 5 different blocks of the HTML5 form when the user touches from a Tablet and each block when touched, should pop up a seperate dialog box on top of the HTML5 form to accept values. And when user is done entering the values, The values gets entered back into the HTML5 form.

Attached is the example of the Chart which needs to be converted into HTML5 using Telerik KendoUI HTMl5 controls.

I would be providing you with a similar HTML5 application with most of the flow seen on the above and all you have to do as shown in the video, the way the different blocks of the anesthesia chart gets highlighted and a seperate box opens up to accept the values and the values get put back into the Chart just like shown in the video.
Description of requirements/features: Create a HTML5 anesthesia form

Create different dialog boxes with input controls for each section of the form which is "touchable" on a Tablet

Submit the values back into the HTML5 anesthesia chart back from the dialog box values.(as shown in the video)

And need to use Telerik KendoUI html5 controls.
Extra notes: I would need a mandatory Skype Call and need to define a very urgent timeline, before I give the project to the right candidate. Additionally if the work is professional on this project and be able to deliver quickly, I will re-hire the same resource for 3 more projects on HTML5.
