Joomla Chrono Form/Connectivity

I need to polish up some forms we are using to collect data through our clients website. I have posted a PDF file with the details of the project.

I would like to hear from people who know Joomla and can work with the Chronoform and Chrono Connectivity modules. The modules are active and installed for the site.

Input on ways to make the project display and work better are welcome. I do not claim to have all the anwers and am willing to consider alternate solutions.
