I need an Umbraco Developer to add an upload file function to recruitment website

We need the following delivered:

A) Create a new “application” document type to store all of the data in the existing form PLUS an uploaded CV (could be word or pdf)
B) Create a new application page that looks exactly the same as the existing page (plus cv upload) but writes all data to the document type
C) NOTE: Ensure the new application page picks up the job advert they are applying for
D) After the application is posted we would like the entire application (including cv) emailed to xxxxx@xxxxx

Before work commences we would like to see a brief technical summary of how the solution will be delivered. Any suggestions for improving the application process or providing more editorial control of the page content will be gratefully accepted!

We would like you to create the application page in your own Umbraco project which we will merge into the live project. As such we require you to do the following:

Build new document type in live Umbraco site
Replicate the document type in your site
Build controller in your site
Build template and page in your site
Build CSS and Javascript in your site
Demonstrate it working remotely

Send us all of the above and we will import into our Umbraco project.

NOTE: We will provide access to the Umbraco project so you can access existing CSS etc.
