I need a website designed along with some back end

The premise behind the website is to provide a website service which will match up surgeries with patient's in their local area who require emergency treatment on a 'same day ' basis. NOTE - The website won't go live for patients until I achieve the relevant number of surgery sign ups, so I only need it live for surgeries to sign up– then it can go live and let patients search but I want the site fully operational at test level. Must be able to incorporate SEO and advertisements fully when it needs to go live

I'm very open to the design and functionality but at a minimum I'm hoping you can provide me with -

A basic website designed and built and hosted. Clean and professional in style and incredibly user friendly with probably only 4/5 pages that are easily navigated. MUST BE MOBILE PHONE LAYOUT COMPATIBLE

A dashboard for registered surgeries ( that can be opened from their computer desktop) where certain info they fill in each morning can be extracted (ie available appointment times/contact info ) at the same time each day, so a list can be displayed to the searching patient, of the number of surgeries who have available appointments that day. All the registered surgeries info needs extracted at the same time ie 9.30am each morning

Geo locating to match the patients UK postcode location with the surgeries nearby who have available appointments. A mileage parameter search as a drop down menu for the patient to select

An admin dashboard to provide sign up/ revenue info,to add/remove/edit surgeries dashboards

A pay/ merchant service implemented on the website to enable patients to pay website but also for patient's to receive refunds as there will be a cashback program in operation.

Linking of company Facebook page and other social media sites

Analytics – website visits etc

I have the surgery dashboard and the admin dashboard layout already mock designed
I also have a flowchart created which will show how the whole process should work from both the patient's side and the surgery's side to give you a better understanding
