I need a professional email programmed for Mailchimp today

Hi I need an HTML email programmed to send from Mailchimp which will have a button whereby the recipient of the email could click and they will get sent to an Update Profile form whereby they will opt in to receive emails from our restaurant Las Iguanas.

Once the code is done, I would need the programmer to use our mailchimp account to set up the email so it renders properly in an email clients.

I need this email programmed by the end of today (12/06) already have the graphic design in JPEG... SEE ATTACHED. It will need to be sliced and programmed with HTML to send from Mailchimp.
