I need a new footer include designed including lightbox overlay

Kind of development: Customization of existing website
Num. of web pages/modules: 4 modules
Description of every page/module: 1. Call to action include - text plus button
2. Newsletter sign up include - email address field plus button
3. Footer - links, text, images
4. Lightbox overlay - clicking a button triggers a faded overlay with a simple email capture form which links to campaign monitor - campaign monitor code provided
Description of requirements/features: Summary of requirements
• 3 includes created exactly as shown in the design
• Each include spreads across the entire width of the page
• PHP includes are created using existing global.css file
• A lightbox overlay tool is created which can be re-used in other areas throughout the site
• The lightbox and the form are customisable
• The form links directly to campaign monitor using the code provided
