HTML Web Contract Form Process

UPDATE***I would like this to be a PDF form instead of .doc or .docx. I want the customer to fill out the pdf form and when they click submit or send it will send an email with PDF attachment with their information filled in. If possible, I would like it to automatically insert their IP address and date and time next to each of the signature fields. For the signature boxes, if possible, it would be nice for the customer to be able to choose a few different fonts for their signature which make it more personal to them. An example of how I would like the signature to appear on the transmitted pdf is also uploaded for you to view.****

I have a real estate website that involves clients signing a listing agreement. We have been using an HTML form process in which the person just types in their name in a box and it sends us a text email with the name and other contract data, but the formatting is usually messed up which means we end up wasting time copying it into word in order to reformat it and print it out.

Ideally, we'd like the online form to generate a .doc or .docx file which would attach it to an email and send it to us and have the persons signature appear in a font that they choose. This way we can just download and print out the attached doc and formatting will be perfect. Please let me know your thoughts and cost.
