Google sheets form / survey by staff

Hi there
We believe we may need to use a Google sheet form or similar to add more detail to a customer enquiry sheet / table that has a reference number assigned to it keying the data together. Our staff need to use the form or tool to structure data back into sheets or another DB we have during phone calls we have with customers. We are expecting to add details like if you selected an Audi A4 on our website - would you prefer a 3 or 5 door would be asked on the phone and captured. We are happy to work with the question part once the structure is in place. We also need to capture tie stamps on the data as it is added or edited and the name of the person making the changes.
