Forms for completing on tablet PC

I have a number of forms that I would like to load up onto my tablet PC and write over the top of them. There are a couple of options -

1. The form simply captures my handrwriting or
2. The form turns it into text

Please can you quote either for:
A. A piece of software that allows me to create as many forms as I like
B. A per-form quote (some forms like the inventory one I am using can run to around 10 A4 pages+, so I need to be clear if you are quoting per form, or per page)
