Form creation

I want to have the facility to send a form to a customer base. The form would have a limited number of fields including email address, phone number, website address, plus two additional pieces of information (just text).

I want to integrate this into my website which has a CMS system in which I am able to paste HTML code.

I'm not sure how the results would be submitted or stored so would need advice on that. Ideally the information would be collated in a CSV file.

It's a one-off form: that is I'm unlikely to need it again for at least a year.
