Fix a php video upload page which doesn't seem to be working

I own an existing website which allows users to upload a short video associated with their user profile. Once they upload the video it gets sent to my yahoo account. I then download it and convert it to a flv file and upload it to my server - and then it displays on the webpage.

However, the problem is that the videos don't seem to be arriving at the yahoo email inbox anymore. I receive a notification email saying a video has been sent and there is a paperclip suggesting there is an attachment, but one doesn't seem to be there (is it a yahoo mail problem?)

Anyway, I'm not the web designer and she isn't available to sort this out, so if any body could fix this page - or create a new page which does the same thing (although this seems like a waste of time to me) I would be really appreciative.

I suspect this will only take a few minutes for the person to sort this, but I'll pay for an hour - although please let me know if you think this will take you more than an hour.

I will send you all the relevant pages and code which is in use once I've accepted a freelancer.
