EXTJS and PHP Expert Needed URGENTLY

Hi,

I Am looking for a EXTJS and PHP expert who knows pretty much anything and can build anything onto a existing system. The system i want to build more modules for is www.group-office.com. This system is a open source system and need this module built for it. I am lookin to get this done within one week and starting today. You will program and design module how i want.

Urgently looking to start work now.

FEATURES I WANT IN MODULE:

CUSTOMER VIEW/FEATURES:
i want a unique ajax form for my website which when they select a branch location it then shows what services that branch location offer and then once they select that service it then shows real time calendar of available slots and times available for that month.
Fields to show in form:
Branch Location (Dropdown)
Services for branch (Dropdown)
It show calendar and available slots which reads from DB.
First Name
Last Name
Telephone
Mobile
Address (UK style)
Other Information Box
Option to pay now online or at the counter (Two Tick Boxes)
All the above info must be sent to the booking module for staff to know what the customer selected.
confirmations will be sent to the customer and the staff member. also i want another email confirmation which chases the customer 24hours before appointment.
Customer will be able to pay for certain services they select under the branch as not all services will have prices.
Customer will not have a login at the moment

STAFF FEATURES:
View calendar weekly, monthly and daily in REAL TIME.
assign how many appointments they can do in a day which will show availability slots on the website form
can resend confirmations
WIll have own logins where they view there own customers in a list form showing fields which are set in the website form.
Staff will only view appointment if the payment was successful if a service had a price. If service does not have a price then shows instantly.
Quick View on dashboard to show latest appointments different colour so calendar must be colour coded.
They must be able to have button to say appointment complete to mark as done and ones in green or red to show appointments coming up in calendar and dashboard.
staff can block out slots when not taking appointments

ADMIN FEATURES:
i want admin to select which department the staff member will be put under e.g. staff member one can be a advisor and staff member two could be a pharmacist.
Can view anything
Assign Permissions in the default group office
can override appointments
can edit all confirmation emails admin can only
There will be a section under address book called "Website Bookings" which will captcha the customers details as well, so the details get store in there and also shows the details under a appointments button under booking module.
Other settings but can't think go more right now.
Assign services to each branch with a pricing and some which are free
