General information for the website: Need to add one custom feature to a one-page website

I want to add one feature:
At the bottom (Join Us/Sign Up section) I want to have a textbox (with the same aesthetic colours/font as rest of the site) that allows the user to enter a code ... if the code is recognised then clicking Sign Up takes them to a URL.
If the code is not recognised it says 'Sorry but that code is not valid. Please contact us and your broker, per below, and we'll get that sorted for you'.
