Create, code and test a minisite that produces/emails personalised results

Description of every page/module: 1) Welcome page, incorporating 1st page of survey, leading to 4 others pages of survey, leading to customised results page
2) 'About us' page
Description of requirements/features: Hi, I'm looking for someone to design, code, set up and test a minisite that hosts a digital questionnaire. The site backend/spreadsheet will score the responses to the questionnaire and present the user’s personalised results, both on the results page of the website and in a form that is emailed to them.

The results should be graphically displayed as well as displayed in text format - eg a spider diagram.

You will need to register and set up the domain name as part of the project.

You are welcome to use a theme as the basis for the web design, or any third party software to handle the questionnaire and results. If so, please tell me what systems you'd like to make use of, and whether there will be any costs (ongoing or one-off) to access these tools.

There will be a second phase to this project (adding some functionality), which can be discussed on successful completion of this first part.
