Convert Excel File into Webpage

This project is to fully convert an Excel file into a working webpage. The attached Powerpoint file gives the exact guidelines for how the working webpage needs to function.

Since the scope of the project is so well defined, and the exact spreadsheet and spec document are attached to this job, I am requesting you give me an exact price to deliver the completed working webpage - i.e. a price for the entire project. This job was previously posted for a quote, but now I am re-posting with the actual spreadsheet, so more exact quotes can be given.

Here are some additional details:
This job is to take certain portions of an existing, completed Excel spreadsheet and convert it into a working web page using javascript. The page will have 4 sections to it.

Section 1 involves 7 lines of user input with drop-down menus, and a single output that adjusts dynamically based on the selection (mimicking the functional Excel).

Section 2 involves 4 input lines with drop-down menus, and a single output that adjusts dynamically based on the selection.

Section 3 involves 6 fields with checkboxes, with each checkbox being checked or unchecked affecting a single output.

Section 4 involves 4 text input fields, with an output for each of the fields based on the entry.

There is only basic math involved (multiplication, division, addition, etc). The page will need to have a "print" button which prints all of the results onto 1 page.
