Configure my "contact us" WordPress forms

I have ten WordPress websites. Each website is the exact same Theme, with same email plugins.

I require all ten emails configured, so they ...

1). Each arrive to my MS Outlook within an attractive form (see attached). The website already does this in another "contact us" form on the website, so this should be easy enough to duplicate.

2). I require so upon sending a message in "contact us", for a popup message to appear for the user to see (see attachment). The website already does this in another "contact us" form on the website, so this should be easy enough to duplicate.

There are also three other small jobs that also require doing for my email setup, please see attachment for full details.
