Code online quiz and capture emails

Create an online quiz (using our questions) which does the following:

a) shows a different results page for 4 different outcomes (with those outcomes depending on which question path the respondent selected throughout the quiz)
b) captures the respondent's name and email address AND sends this directly to our email database on the Get Response t email marketing system (so all respondents are immediately in our system)
c) captures the detail of their outcome (ie: which of the 4 outcome paths reached by each user) and includes this in the information integrated with our Get Response account.

NOTE: we are also very happy to accept bids which can achieve this by using any preexisting quiz system, so long as the email integration can be achieved.
