Basic Incentive Program Website build

Hi all,

I need a website built for an incentive program I will be running for my start up.

Once you understand the intent, the algorithm and script should be simple enough to implement. There may already be code out there that can be integrated. A full design will be provided.

Development requirements are detailed in the attached.

Deadline: ASAP
