Automatically send email on request

We have a website that has recently be generating a lot of traffic. Customers will often call us and ask us for more information. This has become very time consuming.

I want to add an option on the page for "Request More Information". When the user puts in his name, phone number, and email Address I would like a personalized message sent to him including several files.

I will create the content of the email as well as the attachments.
