ASP Quiz Form Update

I'm rebuilding a website for a client and part of the project includes re-using an online test. It has a series of screens to input answers using Flash, while the functionality of the form - which basically totals correct answers and sends and email notification at the end, is written in asp.

I don't think the Flash should need any work, but I'm having difficulty getting the final email notification to send.

On the final page of the test are fields for First Name | Last Name | Email | Select of different schools to send the test entry to. But on submit I'm getting errors.
