Adding Fields of MYSQL/PHP

Description of requirements/features: We have a custom tracker for our work "Computer Repair"

We are looking for someone who can Adding fields to a mySQL database and form, then making them display on several pages.

I'm looking for a tick box for yes or no. If yes then a box is enabled when a comment must be left. this will be the same on both fieleds

We did not design the software so have little documentation.

Once complete we would like it to be fully documented in the code and on paper how to was done in case it need some again.

I will be setting up a demo for the work. as I won't really what to work on a live system.

Knowledge of MySQL and PHP is a must.
