Adding competition onto website

Hi,

I need to add 2 competitions onto the homepage of my website.

these competitions need to be bright and attractive for users to want to enter.

I have images and wording for the 2 competitions so I need the box to be able to have fields for users to input -

- their name (first and last)
- Address
- Email address
- Telephone number
- How they heard about my website
- Sports they like to play (from a drop down list)
- Need the 'are you human box'

they need to be 2 separate boxes.

Once a user enters the competition, I need an email sent to me and also an email sent to the competition entrants email address.

The competitions must be live by 14th September.
