    What platform is your website built on? Are you using a plugin for the form?

    Without more information, it's not possible to quote accurately at this stage, understandably.

    Be very careful in accepting any proposals you have had so far as they cannot possibly understand what they have quoted for without full details of the work (very common for offshore developers to say - "I can do this".
