3step signup form

I need a 3step signup form. Approx. 3 fields on each step. Data must go to MySQL tables and also sent via email! I'd prefer it to be done in PHP but you can also do it in whatever language you wish but the database must be MySQL as I'll need later PHPgrid to be set up there to manage all this. If you have any software in mind similar and simple to edit database tables then of course open for suggestions there, too.
