3 day project correcting and developing further form

I need a freelancer to work with me on the form I'm trying to finish with my current programmer.
Unfortunately, due to lack of commitment, I'm looking for someone who can work with me during the next 3 days (starting Saturday/Sunday) to finish it. I need someone who can be in constant contact with updates, questions, to be sure it can finish asap.

Please review as this consist of some basic HTML, but forms, graphics generation etc.

The form needs an additional 10 pages, some of them will be a copy of the existing pages with some changes, some need to tweak etc. The final output needs to be generated into PDF file. I will be working alongside you with the content etc, checking the outputs etc.
