I need custom Wordpress Plugin

I need a custom WordPress plugin that will work in WP v4.7 and above.

The plugin will create a shortcode that when is added into the post it will display form with 2 fields that are mandatory, email and serial number.
When the user submits this form with a valid email and serial number(serial number will be validated against values stored in DB that you need to implement) hi will get a link to the file that he can download.
Form submission should happen on the same page without refresh (with AJAX call)
It should work similarly to this http://howpolaroidareyou.com/software

The plugin needs to provide the ability for admin user to:
- add individual serial numbers and also able to upload serial numbers from CSV that will be used when validating form submitted by the user.
- ability to add files that will be downloaded when users submit the form, this should support the ability to add multiple files for download when the form is submitted by the user
- admin user should also be able to view logs for each download with what serial number was used to download when (timestamp) IP address of the user who downloaded file
