Description of requirements/functionality: I need redistributable document (pdf or word) that has to be filled with some usual datas (name, last name, birth date, birth place and so on) and one other field that has to contain data that can be grabbed from a public website (aka "university name").
This document will be printed by the user.
When printed the document must show a QR code that collects all the datas filled in the form, so i can scan that QR code and put those data in my own access database.
The document must be easy to change in the future.
The document must work offline and CANNOT use google apis.
