Web form to PDF and Google Drive

General information for the website: Student Contract complete with Sign and email PDF
Description of requirements/features: I am looking to create a form that students can fill in online here is what I would like

- To convert the attached form into an online form into a PHP / HTML based form

- For a PDF copy of the form to be sent to 2 email address 1) the students personal email 2) the students school email

- If possible a copy of the PDF to go into our admin Google Drive account OR if not go into a folder on the web server so we can download all PDFs in one go, note: if on web server will need to be able to clear out with a single button

- must capture students signature using something like this http://rominirani.com/2013/12/02/html5-signature-storage-and-app-engine-app/

- Ideally a list of students student number (captured in form) in a Google Sheet so I can run a Vlookup on this.

Attached is a copy of the file in PDF and word that we want to see at the end I have highlighted in yellow fields that will come from the form and in green fields that will come automatically.

This is NOT the final final form, the only difference will be that there will be an arabic translation next to it, but gives you an idea to get started? I will upload the complete with English / Arabic side by side soon.
